Financial aid often involves submitting additional forms to confirm eligibility, verify information, or document special circumstances. To make the process easier, we’ve organized commonly used forms and links by student type and situation. Review the sections below to find what applies to you—and if you’re unsure which forms you need, our Student Financial Services team is happy to help guide you.
Verification Documents
These forms are designed for students claimed as dependents on a parent’s tax return and for parents assisting their student through the financial aid process. You may download and complete these to verify household income, submit parent signatures, or request updates to your FAFSA data.
Family Size Verification – Dependent ↓ – Confirms household size and college enrollment information for dependent students.
Number in Household and College ↓ – Verifies the number of family members in the household and how many are enrolled in college.
Student Non-Filer Worksheet – Dependent ↓ – Used when a dependent student did not file a federal tax return.
Student Marital Status Form – Dependent ↓ – Confirms marital status for an independent student when additional documentation is required.
Parent Declaration of Nonsupport to Dependent Student ↓ – Documents that a parent is not financially supporting the dependent student.
Student Identity Verification and Certification Form Signed by Notary – Verifies student identity and educational purpose using a notarized statement.
Student Identity Verification _ Signed by Cheyney – Verifies student identity and educational purpose in person through Cheyney.

Independent students typically qualify based on age, marital status, military service, or other circumstances defined by the U.S. Department of Education. These forms help verify your status and financial situation when no parental information is included. Just download and complete.

Special or Unusual Circumstances
If you’ve experienced a significant change—such as loss of income, family separation, or a dependency status concern—you may qualify for a professional judgment review. These forms available for download allow Student Financial Services to re-evaluate your aid eligibility or update your FAFSA data.
Students and parents that have a circumstance that may result in an adjustment to the student’s eligibility such as the following should complete the Special Circumstances form and return to the Office of Student Financial Services for reconsideration of income. Students that have unusual circumstances relating to your relationship with your parents making it difficult to provide their information on your FAFSA application, should submit an Unusual Circumstances form with required supporting documentation.

Special Circumstances Appeal and Instructions ↓ – Requests a financial aid review due to significant changes in income or family circumstances.
Unusual Enrollment History Worksheet ↓ – Provides information about prior enrollment history when required for financial aid review.
Unaccompanied Youth Form ↓ – Documents eligibility for aid consideration related to a parent killed in the line of duty.
Parents Killed in the Line of Duty Form ↓ – Documents eligibility for aid consideration related to a parent killed in the line of duty.
*Tip: Each case is reviewed individually. Be sure to include supporting documentation when submitting an appeal or request.
Recognized circumstances are below:

| Loss of earnings due to a loss of job, change in hours or reduction in pay | Parental abandonment or estrangement |

| Loss of lump sum payment received in 2023 such as severance pay or withdrawal from a retirement program | Death of a custodial parent and no contact or financial support from non-custodial parent |
| Death of a spouse/parent after the FAFSA application was submitted | Human trafficking, as described in the Trafficking Victims Protection Act of 2000 (22 U.S.C. 7101 et seq.) |
| Alimony/child support payments beginning in 2025 | Other circumstances that prevent the student’s from obtaining parental information. |
